Anonymous and Confidential Report

We're committed to fostering a safe, inclusive and ethical campus therefore, we provide you with an anonymous, confidential, and secure way to report any activity that you feel may be violation of law, regulation, or university policy.

Reportable activities include but are not limited to:

  • Conflict of interest (nepotism, misuse of public position or state property, receiving improper gifts)
  • Hazing
  • Discrimination or harassment
  • Risk and safety concerns
  • Fraud
  • Intellectual property infringement
  • Data privacy
  • ADA matters
  • Academic, employee or research misconduct


Title IX and Sexual Misconduct Report

Reportable activities include but are not limited to:

  • Sexual assault
  • Sexual violence
  • Sexual harassment
  • Sexual discrimination
  • Stalking
  • Interpersonal relationship violence
  • Retaliation


Discrimination and Harassment Report

Reportable activities include but are not limited to:

  • Non sex discrimination and harassment based on race, color, religion, national origin, military/veteran status, familial status, and political affiliation
  • Discrimination and harassment based on disability
  • Retaliation


Hazing Report

Reportable activities include but are not limited to:

  • Presumed hazing involving any member of the CSU community including faculty, staff or students
  • Consuming food, liquid, alcohol, drugs or any substance which subjects others to risk of harm
  • Endure physical brutality
  • Endure brutality of a mental nature
  • Endure brutality of a sexual nature
  • Endure any activity that creates a reasonable likelihood of bodily injury to others


General Incident Report

Reportable activities include but are not limited to:

  • Behavior that violates the code of student conduct
  • Alcohol and drug use
  • Physical/Other harm
  • Theft or fraud
  • Unauthorized use
  • Safety violation
  • Property violation
  • Organizational misconduct


Care Report

If you're concerned about a CSU community member or would like to refer yourself, you can submit a CARE report.

Reportable activities include but are not limited to:

  • Barriers an individual is experiencing related to their wellbeing, mental or physical health
  • Financial insecurity including housing or food insecurity
  • Academic concerns
  • Interpersonal concerns
  • Violence
  • Anything that may be interfering with the individual's educational goals


Do not use this site to report events presenting an immediate threat to life or property.听 If you require emergency assistance, dial 9-1-1 or 8-9-1-1 from any campus telephone.听 Cell phone users should dial 9-1-1 and tell the operator to connect them with CSU Police
